How do I cancel my appointment?

If I am sick, may I get an appointment on the same day?

If you are having a medical problem and you would like to see the doctor on the same day, please call the office early in the morning and we will be glad to work you in during the course of the day. Please be aware that there may be a wait as we work you in to see the doctor.

How may I request a medication refill?

You may request a refill medication by either calling our number at 863-422-0032, or request in our secure patient portal at any time. Please be aware that some refills are only possible after an appointment with the doctor to address safe use of medications. Also, refills for sedatives, pain meds, and controlled substances cannot be done without a patient appointment and documentation.

How do I get my lab results?

Lab results are reviewed at your follow up office appointment where the doctor will go over the results in detail with you. There you will have opportunity to address any concerns about the results in an informative fashion and in a way that you will understand.

How do I get my medical chart transferred to your office for ongoing medical care?

You may complete a release form at the office.

What is your payment and financial policy?

Co-payments and deductibles are due at time of visit. We accept VISA and Mastercard, checks and cash. If you are using health insurance to settle your account, you must present your current insurance card at each visit. Please inform us of any changes prior to your visit. Your insurance company requires us to collect any applicable co-payments at the time of service.

MDY Primary Care Medicine, PL has agreements with several insurance companies, which require us to bill them for services provided to you, and accept as payment the amount specified in the agreement. You will be responsible for all amounts not paid by them, including amounts denied, applied to deductible, or considered non-covered as permitted by your insurance.

We will file an initial claim based up the information that you have provided to us. Under state law, your insurance company has 30 days to process the claim, request more information, or deny the claim and notify us of the decision. If they have not notified us within 90 days of the date of service, it will be assumed that your insurance coverage is no longer in force and the unpaid balance will be your responsibility.

Past-due accounts cost both time and money. Therefore, patients with delinquent accounts will be required to make payment at the time of service. If you are unable to make agreeable payment arrangements, we will be glad to reschedule you appointment. Accounts older than 90 days may be sent to a collection agency. If an account goes to collections, the patient may be dismissed from our practice and will have to seek their health care elsewhere.

Returned checks due to insufficient funds or closed accounts will be charged $25.00 non-sufficient fund fee. Patient will be expected to pay by cash or credit card for the returned check plus fee. Any future checks will not be accepted thereafter.

Will insurance cover botox or any other esthetic procedures done in the office?

All cosmetic procedures or cosmetic medicine in Davenport Florida that is performed or prescribed in our office will not be considered billable to insurances. These are to be paid by credit card or cash only, and due at the time of service.

May I see the doctor for a medical problem and a cosmetic procedure the same day?

In order to service the patient effectively, a patient will be seen for medical issues on separate appointments, and not at the same appointment as a cosmetic procedure. This ensures careful attention to each of these issues and serves the patient in the best way.

How do I schedule an office visit for a cosmetic/esthetic procedure?

At this FREE consultation appointment, you will be given opportunity to discuss your concerns and a treatment plan will be mapped out to enhance your natural appearance and give you the desired results that you are wanting to achieve. Dr. Yee will go over the possible options which may include any or all of the following: Botox injections, dermal fillers, sclerotherapy for spider veins, mesotherapy for dissolving fat or reducing stretch marks or cellulite and more.
